CareChart केयरचार्ट

The file you carry to every appointment.

CareChart keeps your prescriptions, lab reports and daily readings in one dated record — on your phone, in your name. Written in English and Hindi.

Example of a CareChart record: date, time, entry type, reading and details.

This is the actual shape of a CareChart record — every entry dated, timed and kept in the order it happened. The reading above target says so in words. It is not coloured red, and nothing is scored, graded or flagged as good or bad.

What it holds

Everything that currently lives in a plastic folder.

Photograph a prescription and CareChart reads the medicines off it, then asks you to check every one before it saves anything. Upload a full-body checkup and it pulls out each test, keeping the reference range printed on that report with that result.

Prescriptions

Photographed, read, and confirmed by you before it counts.

Lab reports

PDF or photo. The original file is kept exactly as you uploaded it.

Blood sugar

By reading type — fasting, after breakfast, after dinner, bedtime.

Blood pressure

Systolic, diastolic, pulse, position, with the date you took it.

Weight, SpO₂, pulse

Whichever you choose to keep. The rest stays out of your way.

Doctor visits

Who you saw, why, what they advised, when to come back.

Medicines, by hand

Type one in yourself when there is no prescription to photograph.

Walking & exercise

Only the figures the machine showed. Nothing is estimated.

Because every result is filed under the date it was taken, the same test from different years lines up in one row — including reports you upload years later.

Test10-Jan-202610-Apr-202615-Aug-2026
HbA1c8.47.66.9
Triglycerides290235170
Creatinine0.90.91.0
Vitamin D182738

Each figure keeps the reference range printed on its own report. A newer report never rewrites an older one.

New

Your medicines, with the dose for each time of day.

Add a medicine yourself, in order, whether or not there is a prescription to photograph. Each one carries a separate dose for morning, afternoon, evening and night — because one tablet in the morning and two at night is two different facts, and a single "dosage" line quietly loses that.

Today's medicines

The home screen opens on four groups — morning, afternoon, evening, night — so "what do I take now" is answered without a single tap.

It reads 1-0-1-0

Type the pattern from the prescription and the timings fill in. Where a pattern is genuinely ambiguous, it asks you rather than choosing.

Which medicines, when

Ask for any two dates and see every medicine you were on between them, including courses that started long before.

New

One button, and the doctor has the whole picture.

Choose a period — today, the last thirty days, the financial year, or any two dates — then tick what to include. CareChart builds a printable summary with the tables and the graphs for exactly that window, and saves it as a PDF named after you and the dates.

You choose what goes in

Eighteen sections, from current medicines to laboratory trends. Take everything, or only the two a particular doctor asked about.

Graphs for that period

Blood sugar by reading type, systolic and diastolic, weight, walking — each drawn for the same window as the tables beneath it.

Allergies are never left out

An allergy noted three years ago is on a one-day report too. It is the one omission a summary must never make.

Doctor Report

One tap picks the eleven sections a doctor actually reads. You only choose the period.

Or send a link instead

A link that expires in a day and that you can cancel. Your record does not travel through a chat app as a file.

Hindi throughout

Every screen, and the printed report. Medicine names, strengths and units stay exactly as written — in both languages.

Where we draw the line

What CareChart will never do.

Most health apps tell you your reading is high. CareChart does not, and this is a decision rather than a missing feature. A number that is alarming for one person is an ordinary morning for another, and an app does not know which person it is looking at.

  • It does not diagnose, and never names a condition you have not been given.
  • It does not decide a reading is high, low, normal or dangerous.
  • It ships with no built-in "normal range". The only target it shows is the one your doctor set and you entered.
  • It does not change a dose, suggest a medicine, or interpret a result.
  • It does not sell, share or mine your records. There are no trackers on this page either.

What it does instead: it shows the number you recorded, the target you were given, and whether the one sits inside the other. Then it stops talking and lets you take it to your doctor.

Install

It installs from the browser. No store account needed.

CareChart is a web app that installs to your home screen and opens like any other app, including without a network connection for records already on the phone.
